This page gives a list of all the Level 4 limit breaks for the characters and where exactly to find them.
Cloud - Omnislash In order to get Cloud's best limit break, Omnislash, you'll need to head to the Gold Saucer and do a lot of fighting in the battle arena. When you get 32,000 Battle Points (BP) you can exchange them for the Omnislash limit break!
Tifa - Final Heaven To get Tifa's Limit Break, you'll need to head to Nibelheim and take Tifa to her old house. When inside, find the piano and play the correct song. It is below:

When the song is finished, press the start button and Tifa will mention she has the sheet music for the song you just played. She will find a note with it that has the Final Heaven Limit Break.
Barret - Catastrophe Barret's is really simple to obtain. Simply head back to North Corel once Meteor has been summoned. Go inside the middle house on the west side of town and talk to the woman inside of it. She'll give you the Catastrophe Limit break.
Aeris - Great Gospel If you speak with the sleeping old man that is in a cave near Junon when you've fought a number of battles that the last two digits of the number are the same (for example, 299) he will give you a piece of Mythril. This can be used to be exchanged for the Great Gospel Limit Break, which is in a small box.
Red XIII - Cosmo Memory Simply defeat the Lost Number Boss after opening the safe in the Shinra Mansion. You'll recieve the Limit Break after defeating the boss.
Yuffie - All Creation Go through the Pagoda of the Five Mighty Gods sidequest in Wutai. If she defeats her father on the top floor, she'll get the limit break.
Cid - Highwind All you'll need to do is head to the crashed Gelnika that's off the shore of the Gold Saucer once you get the Submarine. Once inside, go to the Cargo Bay and you'll find the item for Cid's Level 4 Limit Break!
Vincent - Chaos Once you have the submarine or Gold Chocobo, head to Lucrecia's cave behind the waterfall in the Nibel area. Go there with Vincent in your party on Disc 2, then revisit on Disc 3 to get the Chaos Limit Break.
